What is Reiki? Reiki is a healing modality from a branch of holistic, complementary therapies which enhance well-being and stimulate the body’s natural ability to heal itself. They are used to promote inner peace and relaxation, having a transformational effect on any presenting imbalances within the mind, emotions, physical body and auric field. The result is a deep sense of calm. What Reiki is not. Reiki, as a complimentary therapy, is not a substitute for medical examination, diagnosis, treatment, or medications. Reiki practitioners do not diagnose conditions, nor do they prescribe substances or perform medical treatment, nor interfere with the treatment of licensed medical professionals. It remains the client’s responsibility to take care and seek medical treatment when necessary. By agreeing to this consent form, you are agreeing that you are responsible for your own health and that you understand the parameters of the treatments being offered. How many sessions will I need? Depending on the outcome of the first session this can be discussed. Some clients need only one session while others desire a course of sessions and subsequent follow-ups for self-care and wellbeing. Reiki is safe for everyone and there are no contraindications as far as we know. Minors (under 18 years old): A parent or guardian must be present during the treatment. Data Protection: We value your privacy and keep your information confidential. We won’t share your details with anyone outside of our service without your written permission. However, we may have to disclose information if a child is in danger or if a client poses a risk to themselves or others. In such cases, we will inform the appropriate authorities. Your data is kept securely for seven years. You can ask for a copy of our data protection policy at any time. Data protection laws allow you to request your notes and have them removed. I have read and understand this, and I ask for Reiki Therapy. I know my practitioner is not responsible for the session outcomes. I consent to use the services of Evelina Campbell from The Cosmic Zen and will pay for them. Client Acknowledgment: I recognise that a Reiki Therapy Practitioner does not diagnose or treat medical conditions and does not interfere with licensed medical professionals. I understand these therapies are not substitutes for medical care. I will inform Evelina Campbell of any relevant changes to my condition, like pregnancy or epilepsy. I will be fully clothed during the session, except for shoes and coat. I understand my information will be used by Evelina Campbell to perform the treatment.
